There is a decrease in spontaneous movement, so that when movement does take place it is slow. As the activity takes place, the slowness is reduced. At the beginning of movement akinesia (loss of movement) is marked and so the patient ‘freezes’. Akinesia and bradykinesia lead to a variety of symptoms and signs, which are dependent on the body part. Falls are common and balance is impaired, although gait disturbance and falling are not prominent early symptoms of idiopathic Parkinson’s disease. The patient does not swallow as often and so the saliva collects.

The changes in posture lead to a lack of stability and poor communication (Bull, 1987). Changes in posture play a large part in communication and conversation. In conversation and social interaction unconscious movements reflect feelings and responses. Postural changes give messages to other people. Verbal communication is unconsciously influenced by postural changes in others. The disease progression leads to sideways lean. The chin may be slumped forward on to the sternum. There may be an inability to stay upright.
